How is this product different from the keratin complex infusion therapy?

The infusion is the most effective leave in keratin for hair. it contains keratin as the first and primary ingredient and has no hydrolyzed wheat protein.

Is this product sulfate and sodium chloride free? I would be using it on my hair extensions.

Does not have it. This does not actually apply to this product. Sulfate and chloride present in shampoos and not conditioners

Does this product contain formaldehyde?

Yes, This is not a Keratin Treatment, therefore it will not straighten your hair. This is a keratin replenisher for keratin treated hair and it can be used to simply remove frizz and add shine.
